Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An initial inspection was conducted on 5/26/21. The director was contacted by telephone to initiate the virtual inspection. There were no children present during this inspection. Staff files, sample forms, and background checks were reviewed prior to the virtual inspection. Information gathered during the inspection determined non-compliance with applicable standards or law in the area of administration and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. There was peeling paint on the door frame of the 1 y/o classroom.
2. The cove base in the girls restroom was separated from the wall.
3. The HVAC unit outside the 2's and 3's classroom exit doors did not have a grate with safe spacing that would prevent access to the fan.

Plan of Correction: 1. The director will paint the door frame.
2. The director will secure the cove base and put in a maintenance request to the church.
3. The director will install fencing that will prevent access to the HVAC unit.

Standard #: 22VAC40-185-330-B
Description: Based on observation it was determined that the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
1. There were weeds/grass growing in the resilient surfacing in the fall zones of the climbable playground equipment. This can reduce the resilient surfacing's impact attenuation.
2. Director confirmed that there were weeds/grass growing in the resilient surfacing during this inspection.

Plan of Correction: Director will remove weeds and grass from resilient surfacing.
