This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 05/24/2021 and concluded on 05/28/2021. The director was contacted by telephone to initiate the inspection. There were two children present and two staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-580-H Description: Based on a review of records and interview, the vendor did not ensure two out of two staff members who work directly with children completed the department's health and safety update course annually.
Evidence: 1) The most recent Annual Health and Safety Update Training in the record for Staff #1 was completed on 09/26/2019. 2) During interview, Staff #1 reported the Annual Health and Safety Update Training was not completed annually in 2020.
3) The most recent Annual Health and Safety Update Training in the record for Staff #2 was completed on 06/04/2019. 4) During interview, Staff #1 confirmed Staff #2 did not complete the Annual Health and Safety Update Training annually in 2020.Plan of Correction: Per the Vendor: Staff #1 and Staff #2 are working on completing the training and will have it completed by 06/04/21. The trainings will be taken annually moving forward.
