In order for the application to be deemed complete, the program will need to submit the local agency permits and Building Evaluation Form.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An Initial inspection was initiated on June 3, 2021 and concluded on June 3, 2021. The Director was contacted by telephone to initiate the inspection. There are no children present. The Inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed records and staff records submitted by the facility to ensure documentation was complete. The facility had rooms set up for review but the staff have not put together the cribs. The program is not planning to provide meals at this time and discussed storage of children's lunches. The outside are for the infants is being improved upon and outdoor play space does have shaded area for summer months.

Standard #: 22VAC40-185-540-C Description: Based on interview and observation, the first aid kit did not contain a minimum of two triangular bandages. Plan of Correction: We will purchase another.
Standard #: 22VAC40-185-540-E Description: Based on observation and interview it was determined the facility's nonmedical supplies did not include a working battery operated radio and a flashlight. Plan of Correction: We will purchase and keep at the facility.
