Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 6/1/21 and concluded on 6/3/21. The Director was contacted by telephone to initiate the inspection. There were 5 children present and 2 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Bases on record review and interview it was determined the center did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #3, #4, and #5 all had documented food allergies and are currently enrolled at the center. There were no allergy care plans for these 3 children.
2. When asked if they had allergy care plans for these children staff #3 responded "No I do not, We do not serve Peanuts, and there is no medication on-site".

Plan of Correction: Center will ensure that there is a written allergy care plan for each enrolled child diagnosed with a food allergy.
