-Discussed providing children with a blanket when napping on a cot -at times other than the designated rest period.

Reviewed Standard 340.D
-In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present.

Comments:
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/24/2021 and concluded on 06/03/2021. The director was contacted by telephone to initiate the inspection. There were 38 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 3 staff records and 3 children's records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before attendance or within one month after attendance.

Evidence:
1. The physical exam for child 1 was obtained on 06/02/2021 which was more than 30 days after the enrollment date of 01/05/2021.

Plan of Correction: Going forward all children will have a physical on file prior to enrollment or within 30 days of enrollment.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning agents shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. A can of aerosol spray air freshener was observed sitting on the toilet tank lid in one of the children's restrooms.
2. A spray bottle of bleach/water solution was observed stored on a high shelf in the hall way located outside of the restroom used for diapering.

Plan of Correction: The air freshener and bleach/water was locked up during the inspection and will remain in a locked place when not in use.

Standard #: 22VAC40-185-550-C
Description: Based on observation and interview, the center failed to ensure that shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Evidence:
1. The shelter-in-place procedure was not posted on the facility evacuation map.
a. The program director confirmed that the shelter-in-place area/procedure was not indicated on the posted evacuation map.

Plan of Correction: The shelter procedure will be posted.
