Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was conducted on 06/07/2021 and 06/08/2021. A complaint was received by the department on 05/20/2021 regarding allegations in the areas Supervision, Forbidden Actions and Behavioral Guidance. There were 77 children present and in the care of 17 staff members. The Director was contacted by telephone to initiate the virtual investigation. Three staff members were interviewed, one classroom was virtually observed and the records for one staff member and one child were reviewed.

The evidence gathered during the investigation supported one of the allegations and the complaint was determined to be valid. One violation was issued.

Complaint related: Yes
Description: Based on interviews with three staff and confirmation from staff, the center failed to ensure the care and protection of one child by not preventing the child from running out of one classroom.

Evidence:
1. Staff Member A reported that on 05/14/2021 she was in the hallway when Child #1 opened the door from the inside of Classroom #1 and ran out into the hallway. She was able to return Child #1 to his classroom.
2. Staff Member C reported that on 05/14/2021, she was teaching in Classroom #1 when Child #1 opened the door and ran out of the room and into the hallway. She reported that she watched him leave the room but was not close enough to stop him. Staff Member A was in the hallway and was able to return Child #1 to the room.

Plan of Correction: The child disenrolled as of June 16, 2021.

For future corrective measures:
1. We will use a Safety Plan that has been instituted by COA. This is a positive support plan that uses strategies and tools to prevent or avoid problem behaviors. This plan involves parents and teachers who work together in a collaborative effort to set goals for the child who is having challenging behaviors.
2. If we have a child who attempts to run out of the classroom in the future and the teacher has implemented the safety procedures, we will assign a second teacher to the classroom to be close to the door to try additional positive strategies from the safety plan. If the child tries to leave the classroom repeatedly, parents will be notified.
