Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including document review and interview.

A monitoring inspection was initiated on 7/20/2021 and concluded on 7/21/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 42 children present, ranging in ages from 2 months to 12 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center did not obtained documentation of additional immunizations once every six months for children under the age of two years.
Evidence: The most recent documented immunizations for Child #1 (15 months old) are dated 8/3/2020.

Plan of Correction: I'll contact the family today and ask for an updated form.
