Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A complaint inspection was initiated on 06/21/21 and concluded on 07/22/21. The director was contacted by telephone to initiate the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-370-5
Description: Based on interviews and a record review, the center did not met all play space requirements by providing infants, who cannot turn themselves over and are awake, a total of 30 minutes each day of time on their stomachs, to facilitate upper body strength and to address misshapen head concerns.
Evidence:
During an interview on 06/25/2021, infant staff #1 stated she gives infants in her care, 3 minutes of tummy time each day. When asked if she knew how long infants are supposed to receive time on their stomachs, infants staff #1 replied, "No."
Infants, who cannot turn themselves over and are awake, shall be placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns.

Plan of Correction: All infant teachers were retrained on company and state tummy time policy. Management will do periodic inspections to ensure compliance. DM will periodically inspect for compliance.

Standard #: 22VAC40-185-420-D
Description: Based on interviews and a record review the center did not provide opportunities for parental involvement in center activities by providing a daily record of infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.
Evidence:
During an interview on 06/25/21 infant staff #1 stated she does not record the amount of time each infant spends on their stomach while in her care.

Plan of Correction: All infant teachers were retrained on company and state tummy time policy. Management will do periodic inspections to ensure compliance.
