Technical Assistance:
Discussed ratio standards during nap time, physical plant, out of state background checks, employee start date and TB test result timeline.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .


A monitoring inspection was initiated on 07/20/2021 and concluded on 07/26/2021. The director person was contacted by telephone to initiate the inspection. There were 62 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-350-E-1
Description: Based on record review, the licensee did not ensure there was one staff member for every four children.

Evidence : During the inspection at approximately1:30pm there was two staff and nine infants .The youngest infant was 4 months old. The ratio should of been 1 staff member for 4 children.

Plan of Correction: Center director will ensure the infant room will always maintain a 4:1 ratio. Center Director will ensure that 30 minute head count checks are being conducted to ensure ratio is properly maintained.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and staff interview, the licensee did not ensure there was a copy of results of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to employment.
Evidence: The director confirmed Staff #3 lived outside of the state in the past 5 years and the center did not have sex offender results prior to employment.

Plan of Correction: The center completed the out state sex offender check on July 26, 2021 and provided inspector with results.
