Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was initiated on June 14, 2021 and concluded on June 22, 2021. The director was contacted by telephone to initiate the inspection. There were 0 children present and 2 staff. The Inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 staff records, policies and procedures, and the emergency preparedness plan submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on review of documentation, operational responsibilities failed to ensure that the center's own policies and procedures contained all policies and procedures that are required by the standards.
Evidence: A. Information required to be provided in writing to parents prior to the child's first day of attendance was missing the following: 1. Days and hours of operation, holidays, closings and a phone number where a message can be left.
2. Policies for picking up children after closing or when a child is not picked up for emergency situations including but not limited to weather or natural disasters.
3. A policy for communicating an emergency situation with parents.
B. The following policies were not addressed: 1. The center's procedures to confirm absence of a child when the child is scheduled to arrive from another program or from an agency responsible for transporting the child to the center. 2. The center's procedures for identifying where children are at all times including before and after a field trip. 3. The center's policy for long term medication being allowed with physician and parent authorization. 4. The center's policy for methods to prevent use of outdated medication.

Plan of Correction: All policies have been added to employee and family handbooks.

Description: Based on a review of policies and procedures, the center did not ensure the written playground safety procedures had all required components.
Evidence: The center did not have written safety procedures to include staff positioning in strategic locations or a method of maintaining resilient surface.
