This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated on 08/02/2021 and concluded on 08/10/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 21 children present with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of
three child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable
standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-160-A Description: Based on a review of three staff records, the center did not ensure one staff member submitted documentation of a negative tuberculosis
(TB) screening within the required time frame.
Evidence: 1) The record for Staff #1, hired on 07/12/21, did not contain documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.Plan of Correction: Per the Center: "The employee has now submitted the results. She had stated originally that she had negative results on 1st day, but left them at home. We will not allow any employee to work until we have them in hand."
Standard #: 63.2(17)-1720.1-B-2 Description: Based on a review of three staff records and interview, the center did not ensure one staff member obtained a fingerprint based background check determination letter prior to employment.
Evidence: 1) The fingerprint determination letter in the record for Staff #3, hired on 05/19/20, was dated 06/11/20. 2) During interview, a member of management confirmed the center did not have the results of a fingerprint based background check for Staff #3 prior to hire.Plan of Correction: Per the Center: "Though this staff member filled out paperwork before the fingerprints determination letter was received, the employee was not on payroll and not working with children prior to the letter being received. We will not allow employees to sign paperwork prior to receiving the letter in the future."
Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of three staff records and interview, the center did not obtain a copy of the results of a criminal history record information check and a sex offender registry check within the required time frame from any state in which one individual had resided in the preceding five years
Evidence: 1) The record of Staff #1, hired on 07/12/21, indicated the staff had resided outside of Virginia in the last five years. The record did not contain the results of an out-of-state criminal history record information check or the results of an out-of-state sex offender registry check. 2) During interview, a member of management reported the out-of-state criminal history record information check and the out-of-state sex offender registry check have not been obtained.Plan of Correction: Per the Center: "We were mistaken about the time frame which the state (out of state) criminal and sex offender search. We thought we had 30 days from hire date and have sent out requests and will do before hire in the future."
