The classroom inside the church building had 2 tiles that had cracked and missing pieces.Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The replacement tiles have already been purchased. They will be installed over the weekend.
Standard #: 22VAC40-185-440-E Description: Based on observation the center did not ensure there was at least 12 inches of space between occupied cots.
Evidence: During nap, two children were observed sleeping on cots that were less than 12 inches apart from eachother.Plan of Correction: The following corrective action was provided by the owner during the exit interview:
Owner will ensure staff keep children resting on cots at least 12 inches apart during nap.
