This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on September 10, 2021 and concluded on September 14, 2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 25 children present, ranging in ages from 2 years 9 months to six years of age, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-130-A
Description: Based on a review of children records, the center failed to ensure that documentation of immunizations were on file before the child's first date of attendance.

1. Child #5's immunizations were dated 09/09/2021. Date of enrollment for Child #5 was 08/25/2021.
2. Administration verified the date on the immunizations and the start date for Child #5.

Plan of Correction: Moving forward administration will not allow children to start at the center until immunizations are received.

Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the center failed to ensure that documentation of a negative tuberculosis screening had been completed with in 21 days of employment.

1. Staff #1 tuberculosis test was dated 09/12/2021. Date of hire for Staff #1 was 08/12/2021.
2. Administration verified the date on the tuberculosis screening and the date of hire for Staff #1.

Plan of Correction: It has been very difficult to get staff into local offices due to COVID to obtain tuberculosis screenings or tests. In the future we ensure that have staff have the tuberculosis screening or test within the required time frame.
