Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on August 13, 2021 and concluded on September 13, 2021. A self-reported incident was received by the department regarding allegations in the areas of Administration, Special Care Provisions / Emergencies and Supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and a violation was issued for Supervision. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interviews and documentation review, the licensee did not ensure all children under 10 years of age shall always be within actual sight and sound supervision of staff.

Evidence: Staff #1 confirmed during a fire evacuation drill on July 19, 2021, Child #1 was left in the classroom without supervision of center staff. Center staff evacuated 8 children from the Toddler room during a fire drill. Once staff and children were outside, Staff #2 reviewed center documentation (digital child roll list), conducted a head count and realized Child # 1 was missing. Staff #3 went inside to the classroom and retrieved Child #1 who was left sleeping in a crib. Child #1 was left unsupervised for approximately one minute.

Plan of Correction: On August 16th we held a training on our emergency preparedness policy. Staff member #2 was released. Staff involved received written reprimands on August 13. All rooms received new color cards.
