Thank you for your time and assistance with the announced inspections conducted for the license modification request. The remote inspection was completed on September 16, 2021, and the on site inspection was completed on September 23, 2021. The purpose of the inspections was to tour the building prior to care resuming at the location, and to obtain measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word `corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-40-E Description: Based on interview, the facility did not ensure the center's activities, services, and facilities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
1. The License is issued for the facility located at 127 Seminole Ct., Charlottesville, VA 22901.
2. The facility submitted a request for a change of location on September 8, 2021.
3. The administrator stated the center opened and accepted children in care on September 20, 2021.
