This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on September 14, 2021 and concluded on September 21, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 35 children present directly supervised by 6 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed in art and individual activity and outdoor play. The inspector reviewed on 2 children records and 2 staff records.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 63.2(17)-1720.1-B-2 Description: Based on staff records, the center did not obtain documentation of criminal history records check prior to 1st Day of employment.
Evidence"
1. Staff #1 (date of employment 8/2021) did not have documentation of criminal history check in record for review during the inspection. The staff member was present at the facility during the inspection today providing care for children.Plan of Correction: Director called OBI to check on Staff #1 status. They said she selected the wrong place of birth so she redid her fingerprints that same day. She was cleared the following day. Staff # 1 was offsite until cleared.
