This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.
A monitoring inspection was initiated on 10/04/2021, a virtual inspection of the program was conducted on 10/12/2021 and the inspection concluded with a record review on 10/19/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 41 children, ages 5 years-11 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 3 children?s records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22VAC40-185-160-C Description: Based on record review and discussion with staff, the center failed to obtain a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing.
Evidence: The most recent tuberculosis test/screening in the record for Staff 1 was dated 03/13/2019; there was not a follow-up tuberculosis screening for 2021 in the record.Plan of Correction: Will notify the staff member and request the staff to get an updated TB test/screening.
