Technical Assistance:
Consultation was provided to the General Manager regarding COVID-19 best practices recommended by the CDC for child care in regards to children in care who show possible symptoms of COVID and when children are able to return to the center after showing symptoms. Further discussed with the General Manager the supervision of children on the playground and cleaning and sanitization procedures.

Comments:
An unannounced complaint investigation was initiated on 9/30/21 in response to a complaint received in the licensing office regarding preventing the spread of disease. An exit interview was conducted with the General Manager on 10/15/21. Based on review of available documentation and interviews conducted, there was insufficient corroborating evidence to substantiate the complaint. Violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on observation, the center did not ensure that when staff are supervising children, they always ensure their care, protection, and guidance. Evidence: On 9/30/21 the Licensing Inspector (LI) entered the playground where the two-year-old class was playing. Both staff members were seated at a table. Two children were sitting next to one another on the ground and one began to throw pebbles into the air. The other child picked up a large handful of pebbles and began attempting to put the pebbles into their mouth. The LI asked the child to stop and directed the teachers over to the children.

Plan of Correction: Staff have been retrained on proper playground safety and supervision policies, and health and safety standards as outlined in the Virginia Licensing Standards.

Complaint related: No
Description: Based on observation and staff interview, the center did not ensure that the diapering surface was used only for diapering or cleaning children, and that it was cleaned with soap and water and sanitized after each use. Evidence: 1. On 9/30/21 the Licensing Inspector observed multiple wash cloths hanging over the edge of the changing table in the two-year-old classroom. Interviews revealed that the wash cloths are used by the staff to wipe off the changing table after cleaning and sanitizing rather than using disposable towels, as required. 2. Interviews revealed that on multiple occasions staff witnessed the changing table in the two-year-old classroom being cleaned and sanitized only after all of the children were changed, rather than after each child.

Plan of Correction: Staff have been retrained on the diapering policy and procedures for Callahan Learning Center, Diapering and Toileting procedures, and Health and Safety standards as outlined in the Virginia Licensing Standards.
