Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on October 29, 2021. A call was initiated with the Assistant Head Start Director and a virtual inspection was conducted. There were 11 children present in one classroom and in the care of two staff members. The inspector reviewed compliance in the areas of administration, physical plant, programming, special care and emergencies. A total of 2 children's records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on the review of two children's records and discussion with staff, the center did not obtain a physical examination report for either child. Standards require the report to be in a child's file within one month of attendance.

Evidence:
1. The records for Child #1 and Child #2 did not contain results of a physical examination. The first date of attendance for both children was 08/11/2021.
2. The Assistant Head Start Director confirmed that the physical exam reports had not been obtained.

Plan of Correction: The staff will reach out to the parent of both children and obtain the reports as soon as possible.

Standard #: 22VAC40-185-70-A
Description: Based on a review of documentation for two staff members and confirmation with staff, the center did not ensure that one staff member had the required health information on file as required by the standards.

Evidence:
1. The record for Staff #2 (Date of Employment 09/23/2021) contained tuberculosis (TB) test results completed on 10/15/2021. Prior to 10/13/2021, the standards required that staff obtain TB results within 21 days of beginning employment; therefore, Staff #2 should have obtained TB results by 10/14/2021.
2. The Human Resource Coordinator confirmed the staff member was late in obtaining the TB results.

Plan of Correction: The staff understands the new change to the TB requirement with the new standards that went into effect on 10/13/2021. Any new staff member must obtain a negative TB screening or test at the time of employment and before coming in contact with children.

Standard #: 22VAC40-191-60-C-2
Description: Based on the review of two staff records and confirmation from human resource (HR) staff, one staff member's file did not contain a search of the central registry finding which is due within 30 days of hire.

Evidence:
1. The record for Staff Member #2 did not contain a completed search of the Virginia central child abuse and neglect registry. The staff's date of employment was 09/23/2021 and the request to have the search completed was sent off on 10/8/2021.
2. The HR Coordinator confirmed that the results of the search had not been received and the request was not sent off until 10/08/2021.

Plan of Correction: If the search of the registry has not been returned within 30 days of the date it was requested, the staff will follow-up with the Office of Background Investigation. Once the search is received, HR will send it to the licensing inspector.
