This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a tour of the program.
A monitoring inspection was initiated and concluded on 11/4/21. The director was contacted by email to initiate the inspection. There were 18 children present and 3 staff. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-140-A Description: Based on record review it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required timeframe.
Evidence:
1. The records for child #1, #2, and, #3 did not contain documentation of a physical exam.
2. Staff #4 confirmed that these records did not contain physical exam documentation.Plan of Correction: Director will request this documentation from parents and ensure each child's record contains documentation of a physical exam within the required time frame.
Standard #: 22VAC40-185-560-G Description: Based on observation it was determined the center did not ensure when food is brought from home the container is dated.
Evidence:
Lunch boxes were observed without dates.Plan of Correction: Director will remind all staff to make sure each lunch box is labeled with the child's name and dated daily.
