Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

An initial inspection was initiated on 11/03/2021 and concluded on 11/05/2021. The provider was contacted by telephone and a virtual inspection was conducted. All designated areas and furnishings for child care were toured virtually. Items reviewed during the inspection include: required posted information, parent pick-up/drop off procedures, emergency maps and exits, the first-aid kit, restrooms, and the daily schedule. According to the floor plan submitted, the facility can easily accommodate the requested 100 children. The ages served will be 4 years 9 months to 12 years. Standards related to supervision and program could not be determined as the facility is not yet fully operational. Three staff records were reviewed. A conditional license will be recommended at this time. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of three staff records and interview, the center did not ensure one staff member resubmitted the results of a follow-up tuberculosis (TB) screening every two years from the date of the first initial screening or testing.

Evidence: 1) The most recent TB screening in the record for Staff #2, hired on 09/08/2017, was dated 10/02/2019. The TB expired on 10/02/2021. 2) During interview, a member of management confirmed this is the most recent TB screening on file for Staff #2. Staff should
resubmit TB test results at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.

Plan of Correction: The staff member will resubmit a TB screening as soon as possible.

Standard #: 22VAC40-185-550-A
Description: Based on a review of the emergency preparedness plan and interview, the center did not develop the plan in consultation with local or state authorities.

Evidence: 1) The emergency preparedness plan reviewed did not have documentation that the plan was developed with local or state authorities. 2) During interview, a member of management stated the emergency preparedness plan has not been reviewed by local or state authorities. The center shall have a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and relocation, shelter-in-place, and lockdown. The plan, which shall be developed in consultation with local or state authorities, shall include the most likely to occur emergency scenario or scenarios, including fire, severe storms, loss of utilities, natural disaster, chemical spills, intruder, violence at or near the center, terrorism specific to the locality, and other situations, including facility damage that requires evacuation, lockdown, or shelter-in-place.

Plan of Correction: The center has contacted local authorities and requested a review of the center's emergency preparedness plan.
