Comments:
An announced initial inspection was conducted on 11/16/2021 beginning at 9:50am and ending at 11:30am. A total of 10 children were present with 8 staff. Observation of all 7 classrooms and the outdoor play area was completed. Record review consisting of 4 children's records, 4 staff records, and 2 board officer records was completed remotely and found to be in compliance. Children were observed interacting positively with staff, reading books, and engaging in various age appropriate free play activities during the inspection. The center's policies and procedures and emergency supplies were inspected and determined to be complete. Inspection of the four cribs on site was completed and meets the U.S. Consumer Product Safety Commission (CPSC) standards. The center has the required space and equipment to serve children ages birth through 12 years. An adequate number of staff are trained in CPR/First Aid, daily health observation, and medication administration training.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, areas and equipment on the playground were not maintained in a safe condition.

Evidence:
1. On the "Big Kid" playground, four nails were protruding from the bottom of the wooden storage structure and one nail was protruding from the red wooden barrier surrounding the large tree that could entangle clothing or snag skin.
2. On the Infant/Toddler playground, at the top of the climbing structure two bolts were observed to be missing from the bottom of the green siding panel. The green siding panel was not secured and could be pulled from side to side creating a space and causing an entrapment hazard.
3. On the Infant/Toddler playground, the plastic exterior of the bench was cracked and falling off exposing sharp edges.

Plan of Correction: Bright Starts considers the health and safety of our children as its top priority. The protruding nails have already been fixed and no longer present a hazard. The play structure is scheduled to be fixed today (11/18) as we expect to receive the two bolts. The bench is scheduled to be removed this week. The repairs have been included in an overall larger renovation project planned by new ownership that purchased the building in October. Once the inside of the building is completely renovated then the outside playground will be renovated to include new resilient surfacing and state of the art play equipment that incorporates motion play, musical play, imaginative play, and other age appropriate play systems. The playground will be designed with a focus on safety, and will be fully compliant with ASTM F1487 and CPSC playground safety standards.
