Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program. A monitoring inspection was initiated on 11/03/2021 and concluded on 11/18/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were no children in care and one staff member present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	22VAC40-185-60-A
Description:	Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contained all of the elements as required by the standards. Evidence: 1. The record for Child 1 did not contain the information regarding allergies and intolerances to food, medication or any other substances, and actions to take in an emergency situation. 2. The record for Child 2 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the address for the second designated person was incomplete.

Standard #:	22VAC40-185-540-C
Description:	Based on observation and discussion with the staff, the center failed to maintain the minimum first aid kit requirements. Evidence: The first aid kit did not contain adhesive tape or triangular bandages.
Plan of Correction:	Will purchase by the end of the week.
Standard #:	22VAC40-185-540-E
Description:	Based on observation and discussion with the staff, the center failed to maintain the nonmedical emergency supplies required by the standards. Evidence: 1. There was not a battery-operated flashlight available during the 11/18/2021 inspection. 2. There was not a battery-operated radio available during the 11/18/2021 inspection.
Plan of Correction:	Will purchase by the end of the week.

Description:	Based on document review and discussion with staff, the center failed to implement monthly practice evacuation drills for the most likely to occur scenarios. Evidence: There was not an evacuation drill conducted in August 2021.
Plan of Correction:	Will schedule with the front desk person to ensure they are done.
Standard #:	22VAC40-191-60-C-2
Description:	Based on record review and discussion with staff, the center failed to obtain the findings from the search of the central registry within 30 days of employment. Evidence: The record for Staff 1 contained a documented hire date of 08/19/2021. There were no central registry findings available for review during the 11/18/2021 inspection.
Plan of Correction:	Will submit a new request.

Description:	Based on record review and discussion with staff, the center failed to obtain a copy of the criminal record check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: The record for Staff 1 contained documentation that Staff 1 had resided outside of the state of Virginia within the preceding five years. The record for Staff 1 contained a documented hire date of 08/19/2021. There was not a criminal record check or a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years available for review during the 11/18/2021 inspection.
