A renewal inspection was initiated on 07/30/2021 and concluded on 08/15/2021. The director was contacted by telephone to initiate the inspection. There were 10 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirements Insert specific requirement and description. 8VAC20-780-80-A
for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in
care as it occurs and 8VAC20-780-40-K the center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness. Standard #: 22VAC40-185-130-A Description: Based on review of two children's records, the center failed to ensure that all immunizations were obtained prior to the child starting the program.
1. Child # 2 started the program on 5/16/2021 and the date on the immunization was 8/8/2021.
Plan of Correction: In the future the director will make sure that all immunizations are obtained prior to starting.
Standard #: 22VAC40-185-140-A Description: Based on review of two children's records, the center failed to ensure that all physical examinations were obtained within 30 days of the child starting the program.
1. Child # 2 started the program on 5/16/2021 and the date on the physical was 8/8/2021.
Plan of Correction: The director will make sure that all physical are obtain prior to starting or within 30 days of starting.
Standard #: 63.2(17)-1721.1-A Description: Based on review of two board officers, the center failed to ensure that all repeat CPS background checks
Board Officer # 2 most recent CPS background check was dated 10/18/2017. The background checks is required to be resubmitted every five years.
Plan of Correction: The director stated that it was sent out and they are waiting the results.
