63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews.
A monitoring inspection was initiated on 11/09/2021 and concluded on 11/29/21. The director was contacted by phone and a virtual inspection was conducted. There were 44 school-age children present, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and 6 staff records were reviewed.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not have fingerprinting processed prior to the first day of employment.
Evidence:
1. The inspector reviewed 5 staff records. Staff #1 was hired 10/26/21 and the fingerprint was dated 11/8/21.

Plan of Correction: The director will work with the Hr director to make sure fingerprints are in the file prior to the first day of employment.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and interview, the center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. The inspector reviewed six staff records. Staff #6 had the required out of state background checks (dated 11/23/21) but not before the hire date of 8/27/21.

Plan of Correction: The director is working with the HR director to make sure all the required background checks are completed when required.
