Thank you for your assistance during this announced inspection. This inspection was initiated by a complaint that was received by our licensing office on September 28, 2021, regarding the allegation of behavior management. The on-site visit was conducted with another government official on October 27, 2021 from 3:30 pm through 6 pm. One school administrator and three program staff were interviewed. The evidence gathered during this investigation did not support the allegation, therefore the complaint is not valid.
Non-compliance with other applicable standards or law were identified during the investigation and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-80-B Complaint related: No Description: Based on record review, program did not ensure that the superintendent's representative was informed timely of an incident.
Evidence:
1. On 9/16/21, Child #1 left school premises and local authorities were contacted for help.
2. Program reported incident to licensing inspector on 10/8/21. Standards require reporting as soon as practicable, but not to exceed one business day.Plan of Correction: POLICY AMMENDMENT: Program administration immediately revised
internal reporting procedures along with reinforcing current regulation 22VAC40-185-(2)-
80-B.
TRAINING: All program staff will receive additional training during Orientation as well as annual training as deemed necessary through ongoing monitoring feedback.
