Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated and concluded on 12/7/21. The director was contacted by telephone and a virtual inspection was conducted. There were no children present, as they had left to go to school. Earlier in the morning, there were 13 children present ranging in ages from five to 12, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on the review of two staff records, the facility failed to conduct reference checks.
Evidence:
There are no reference checks on file for staff #1 & 2.

Plan of Correction: The program director will insure that references are checked for each staff member.

Standard #: 22VAC40-185-240-D-5
Description: Based on the review of two staff records the facility failed to insure that all required training is complete.
Evidence:
No staff member has completed the daily health observation training.

Plan of Correction: The program director reported that a staff member completed the training on12/9/21 and other staff will be trained within seven days.
