Technical Assistance:
Discussed new standards effective 10/13/2021, carbon monoxide detector requirements and the medication forms.

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 12/09/2021. The supervisor was contacted by telephone and a virtual inspection was conducted. There were 48 children present, ranging in ages from infant to 5 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Description: Based on record review, the center did not obtain a copy of the results of criminal history record information check, from any state in which the individual has resided in the preceding five years prior to the 1st day of employment.
1. Staff #1 (Date of hire 11/09/2021) lived in Nevada in the last 5 years and the center did not obtain the results of criminal history record information check prior to 1st day of employment.

Plan of Correction: Center emailed state email regarding criminal history record check and will record response in staff file. State will not send criminal history to center in response in staff file.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of 3 staff records, the center did not have a central registry finding within 30 days of employment.
1. Staff #2 (Date of hire 11/01/2021) did not have the central registry findings within 30 days of employment. Center documents the form was sent on 11/01/2021.
2. Staff #3 (Date of rehire 5/14/2021) did not have the central registry findings within 30 days of employment.

Plan of Correction: Center emailed central registry representative to check status of background check request. Response will be kept in staff file as well as any required continued follow up.
