Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on December 17, 2021 and concluded on December 21, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from five to 12 years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed. New requirements became effective on October 13, 2021. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on a review of children records, the center failed to ensure all required information were in children's records. Evidence: 1. Child #1 & Child #2's file had only listed 1 emergency contact. The requirement is that there be two emergency contacts listed. 2. Child #1's file did not contain any information regarding required health information for allergies and chronic physical problems. 3. Child #2's file did contain a home address for the child or the parents. 4. Staff verified that the required information was not in Child #1 or Child #2's record.
Plan of Correction:	The new administration is auditing all files and will reach out to parents to obtain required information. In the future all forms will be reviewed before the child begins enrollment to ensure all required information is in the child's record.

Description:	Based on review of evacuation drills, the center failed to ensure that a monthly practice drill was held once a month. Evidence: 1. There was no documentation of a monthly practice evacuation drill being completed in October 2021. 2. Staff verified that there was not a monthly practice evacuation drill held in October 2021.
Plan of Correction:	There was turnover within the program at the site and a change in site supervisor. Moving forward the operations manager will be doing spot checks to ensure that evacuation drills are being completed monthly.
