Comments:

Seaford Summary This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on November 30, 2021 and concluded on December 9, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 20 children present, ranging in ages from 5 to 11, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard #:	22.1-289.035-B-4
Description:	Based on record review and licensee interviews, the licensee did not ensure to a obtain the out of state criminal records check and a sex offender check prior to employment The licensee also did not ensure to obtain the out of state search of the central registry check (CPS) as required. The out of state background checks are required for any person who has lived out side of Virginia within the last five years. Evidence: Area Manager confirmed, the following background checks had not been conducted as required; 1) There were not criminal record results or sex offender results available for review for Staff #2 who lived in Illinois within the last five years according to center documentation. Staff #2 (date of hire 8-30-2021). 2) There were not criminal record results or sex offender results available for review for Staff #3 who lived in Illinois within the last five years according to center documentation. Staff #3 (date of hire 8-20-2021). There was not a CPS check available for review from Illinois for Staff #3. Staff #2 and Staff #3 were working during the inspection.
Plan of Correction:	Area Manger will ensure all checks are submitted prior to staff members 1st day.
Standard #:	8VAC20-770-60-C-2
Description:	Based on record review and licensee interviews, the licensee did not ensure to obtain a search of the central registry (CPS) within 30 days of employment. Evidence: Area Manager confirmed, there were not findings of the Virginia CPS available for review during the inspection for Staff #2 (date of hire 8-30-2021) and Staff #3 (date of hire 8-20-2021).
Plan of Correction:	Area Manger will document date sent with a copy and document all communications with state on items not returned in a timely manner
Standard #:	8VAC20-780-160-A-1
Description:	Based on record review and license interviews, the licensee did not ensure that staff have completed a tuberculosis screening (TB) signed by a physician, physician's designee, or an official of the local health department. Evidence: The records for Staff #1 (date of hire 10-12-21) and Staff #2 (date of hire 8-30-21) did not include documentation of a TB screening with a date indicating negative TB results/ screening.
Plan of Correction:	Area Manger will send staff to get a test that clearly defines date, type of test, finding results.
