Comments:
The licensing inspector was on site on Thursday, November 23rd, 2021 to conduct an unannounced renewal inspection and to follow-up on previously cited violations to determine the center's compliance with licensing standards. The inspector arrived at 11:37am and concluded the inspection at approximately 3:20pm. The center's director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 89 children in the direct care of 17 staff members. Additional staff were on site and available to the children in care. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-B
Description: Based on documentation review, observation and interviews, the center did not ensure prescription and nonprescription medication shall be given to a child only with written authorization from the parent.

Evidence: 1) During the inspection, the medication administration log for Child #5 was reviewed. The inspector observed that a prescription medication was administered to Child #5 on 11/17/21, 11/18/21, 11/19/21, and 11/22/21 without written parental permission. The center had obtained written permission from the child's physician, but the written parental permission had not been obtained. 2) During interview, a member of management confirmed the written parental permission was not on file and the medication was administered.

Plan of Correction: The parent was contacted on the day of inspection and notified that their written permission is needed. The parent was requested to sign the form by the end of the day on 11/23/21. Moving forward, the center will ensure each form to administer prescription and nonprescription medications is complete with the required signatures.

Standard #: 8VAC20-780-570-E
Description: Based on observations, the center did not ensure that prepared infant formula was dated and labeled with the child's name.

Evidence: 1) During the inspection of the Baby Birds 2 classroom, the licensing inspector observed approximately 4 infant bottles in the refrigerator that were not dated or labeled with the child's name.

Plan of Correction: Parents label infant bottles each day at drop-off and staff will check and add dates and names if needed. Staff will be retrained on 11/29/21.
