Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on October 12, 2021 and concluded on October 12, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 47 children present, ranging in ages from two years to five years, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
The record for child #1 (date of enrollment 9/23/21) did not have a physical examination.

Standard #: 22VAC40-185-540-C
Description: Based on an observation and interview, the center did not ensure that each first aid kit contained all required supplies.
Evidence:
1. The first aid kit had a bottle of antiseptic cleansing solution which expired in 2019.
2. The provider stated that the bottle of antiseptic cleansing solution was expired.
