Comments:
This inspection was conducted by licensing staff using an alternate remote protocol component and an in person tour of the program.

A renewal inspection was initiated on December 13, 2021 and concluded on December 15, 2021. The director was contacted by telephone and a preliminary record request was conducted. On December 13, 20221, an inspection observed there were 8 children present, ranging in ages from 2 years to 5 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Please call with questions 703-479-4679

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review of children's files, it was determined the center is not maintaining a complete record for child #2.
Evidence:
1. Child #2's file contained one parents work place but not a phone number for the employment place. This file also did not include an emergency contact's address.

Plan of Correction: I completed all the information on the forms that is missing (work phone numbers, addresses second emergency contact information)

Standard #: 8VAC20-780-70
Description: Based on record review, it was determined the facility had not kept complete records or tnew staff hired as the reference checks were not complete.
Evidence:
1. Files for Staff 1 and 2 included a paper indicating references were made over the phone. Staff 1's file included a reference that only listed first name of the reference and was not signed by the person who made the calls for both references. The references in Staff 2's file also did not include the signature of the person who made the call.

Plan of Correction: I did sign(signature of the person who contacted) the reference check forms and did fill in the last names of the persons that are missing.

Standard #: 8VAC20-780-270-A
Description: based on observation, it was determined there were items inside not maintained in a safe manner.
Evidence:
1. Two classrooms were observed with children inside the rooms and both rooms had floor/area carpets. The inspector noted that the area carpet in the two's classroom had not been secured to the floor and the area carpet slid when the Inspector stepped on it. The other room also had area rugs and while some type of adhesive was used to hold carpet, it also was not secure to the floor. One area rug even had edges that curled up which could also cause a tripping hazard.

Plan of Correction: WIll get new carpets and see that they are fixed to the ground(to avoid tripping).
