This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A non mandated monitoring inspection was initiated on 01/10/2022 and concluded on 01/12/2022. The director was contacted by email to initiate the inspection. There were 6 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
New requirements became effective on October 13, 2021. The facility has not yet complied with the following requirements: written procedures for prevention of shaken baby, abusive head trauma, coping with crying babies, safe sleeping practices and SIDS awareness and the new requirements for the orientation for new staff. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard #: 8VAC20-770-60-C-2 Description: Based on review of two staff records, the center failed to ensure that all staff had a CPS check within 30 days of hire.
1. Staff # 1 was hired on 11/1/2021 and staff # 2 was hired on 11/29/2021 and on the day of the inspection did not have a CPS check in the record. There was no documentation that the OBI office was contacted to verify the status of the background check.
Plan of Correction: The director stated that she thought that they were returned and she missed placed them. The director will locate them and send them to licensing. The director will in the future make sure records are complete.
Standard #: 8VAC20-780-550-D Description: Based on review of drills and interviews, the center failed to ensure that monthly evacuation drill was conducted.
The monthly evacuation log did not have a drill for November 2021.
Plan of Correction: The director will ensure that all drills are conducted and documented.
