Thank you for your assistance during this unannounced inspection. This inspection was initiated by a complaint received on 12/10/2021, regarding the allegation of an underage assistant teacher/aide. The on-site visit was conducted on December 21, 2021 from 2:45 pm through 3:30 pm. The evidence gathered during this investigation supports the allegation, therefore the complaint is valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-220 Complaint related: No Description: Based on observation and interviews, the center did not ensure that aides were at least 16 years of age.
Evidence:
1. Staff #2 was observed on 12/21/2021 working with Staff #1 in a classroom with 17 children ages 3 to 12 years old.
2. When Staff #2 was asked her age she stated that she had just turned 15 years old on 12/10/2021.
3. Interview with Staff #2 revealed that she cleans upon arrival and goes into a classroom where needed to work as an aide until the facility closes for the day. Date of hire of Staff #2 is 08/13/2021.
4. Staff #1 and Staff #3 verified that Staff #2 was on the schedule to work in the classrooms as an aide on 12/21/2021.
5. The daily schedule was reviewed to confirm that on 12/21/2021 Staff #2 was on the schedule to relieve Staff #4 at 3:00 p.m.Plan of Correction: Not available online. Contact Inspector for more information.
