Comments:
An investigation was begun on 12/20/2021 as a result of an allegation being received in our office. The investigation was conducted jointly with local agencies.
Based on multiple staff interviews, The preponderance of evidence does not support the allegation. The complaint is not-valid.
Violations cited were found during the course of the investigation, and not part of the original complaint.

Description: Based on a review of records, one staff record does not contain documentation of a Sworn Disclosure statement (SDS) that discloses any out of state residency in the past 5 years.
Evidence:
The Sworn Disclosure Statement on file, is an outdated form and does not list any out of state residency within the past 5 years. This information is not documented anywhere in the staff file that was provided for review. (Staff A)

Evidence:
- Staff A (Date of Hire; 9/7/2021) doe not have a completed CPS search result on file. No documentation was provided to show that the form was mailed off within 7 business days or that any follow up with the Office of Background Investigations (OBI) was conducted to locate the results after 30 days.

Plan of Correction: Reached out to the Office of Background Investigations for update.

Description: Based on multiple staff statements, children have been temporarily reassigned to another group in order to maintain ratios.
Evidence:
12/20/2021 - A child was moved to the School Age room from the Preschool room to maintain ratios. The child was moved when there was another staff person available to go into the room.

Plan of Correction: Will move staff not children.
