Comments:

The licensing inspector conducted an unannounced renewal inspection on 1/18/2022 from 4:20pm to 6:18pm. There were 5 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, medication, special care, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program. New requirements became effective on October 13, 2021. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Standard #:	8VAC20-780-130-A
Description:	Based on record review and an interview, the center did not ensure two of two children's records had documentation of immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The center did not obtain documentation of child #1's (start date: 9/23/21) immunizations required by the State Board of Health before the child can attend the center. The site director stated that the child's parents had not submitted any of the required documentation. 2. The center did not obtain documentation of child #2's (start date: 10/27/21) immunizations required by the State Board of Health before the child can attend the center. The site director stated that the child's parents had not submitted any of the required documentation.
Plan of Correction:	We have already been in contact with their parents and will hopefully receive the documents by end of week. If documents are not received by Friday the 28th, we will not allow them to attend the program starting January 31st.
Standard #:	8VAC20-780-140-A
Description:	Based on record review and an interview, the center did not ensure two of two children's records had documentation of a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The center did not obtain documentation of child #1's (start date: 9/23/21) physical examination. The site director stated that the child's parents had not submitted any of the required documentation. 2. The center did not obtain documentation of child #2's (start date: 10/27/21) physical examination. The site director stated that the child's parents had not submitted any of the required documentation.

The center did not have a record for child #2 (start date: 10/27/21). The site director stated that the child's parents had not submitted any of the required documentation.
