Comments:
An in person renewal inspection was initiated on January 31, 2022 and concluded on February 1, 2022, following all COVID protocol. There were 111 children present, ranging in ages from one month to 5 years with 28 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of 4 staff records, one did not provide a completed sworn statement or affirmation.
Evidence:
The record for staff #1 did not contain a completed sworn statement or affirmation.

Plan of Correction: Staff #1 completed the sworn statement before the inspection was completed.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of 4 staff records, one did not have a record of central registry finding within 30 days of employment.
Evidence
Staff #2 did not have a record of central registry finding within 30 days of employment and no follow up was documented.

Plan of Correction: Director will follow up with OBI to obtain central registry finding and send the inspector the results when received.
