Comments:
An unannounced monitoring inspection was conducted today from 9:30am-12:00pm. There were 64 children (infants-5yrs.) directly supervised by 10 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, staffing and supervision, programming, emergency supplies, and policies were inspected. Children were observed participating in group play, story time and eating lunch. There was an adequate supply of materials for the children.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child #1's record contains an immunization record that was last updated on 4/19/2021.
Child #2's record contains an immunization record that was last updated on 3/18/2021.

Plan of Correction: Maintain updated immunization records from each family to add to child file.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not all contain required information.

Evidence:
Child #3's record did not contain documentation that a staff member viewed the child's original proof of ID.

Plan of Correction: View the original proof of ID for child from family, and sign proper paperwork.

Standard #: 8VAC20-780-90-A
Description: Based on record review, the authorization for emergency medical care should an emergency occur when the parent cannot be located immediately was not signed by a child's parent by the first day of the child's attendance.

Evidence:
Child #3's record contains an authorization for medical care that has not been signed by the parent.

Plan of Correction: Ask family to sign paperwork.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, were not maintained in a clean, safe and operable condition.

Evidence:
1. In the 2 year old classroom, an area of approximately 1 inch by 2 inches of peeling paint was observed on the wall next to the children's cubbies.
2. Rust was observed on the toilet partition panel in the bathroom located across the Gym room, the rusted area measured approximately 12 inches in diameter.
3. In the 3 year old's bathroom, the toilet paper holder was broken, the toilet seat of one of the toilets was loose, discolored and stained.
4. In the 3 year old classroom, peeling paint was observed on the wall next to the classroom's calendar.
5. Peeling paint, 5 inches in diameter, was also observed on the door that leads to the laundry room.
6. In the 4 - 5 year old's bathroom, the toilet paper roll was observed on top of the commode tank, the toilet paper holder was missing.

Plan of Correction: Have area with chipped paint, repainted by facilities team.
Remove partition from bathroom.
Replace toilet paper holder, replace toilet seat.
Facilities repaint area near calendar/laundry room.
Place toilet paper inside of proper holder.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
The storage closet located in the School Age classroom was found unlocked, 14 bottles of cleaning supplies were observed in the closet accessible to children.

Plan of Correction: Fix door handle, and add extra lock to door.
Move cleaning supplies to laundry room.
