8VAC20-780 Physical plant.

A monitoring inspection was conducted in response to a self-reported incident. The inspector reviewed compliance in the areas of physical plant, staffing and supervision, and programming. The investigation included review of documentation, observation and staff interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A
Description: Based upon observation and staff interview, the facility has not ensured that areas of the center are maintained to be safe.
1. There was a small plastic pig and a bear on a low shelf in the front classroom where three year old children were observed playing. The animals are of a size to fit into a choking tube indicating that they are potential choking hazards for preschool aged children.
2. There was a 1/2 inch by 1/2 inch yellow plastic connector block on a low shelf in the second classroom from the front where three year old children were observed playing .The block is of a size to be a potential choking hazard.
3. Staff 1 acknowledged the potential hazards and removed them from the classrooms.

Plan of Correction: The facility responded with the following:
Hazardous toys were removed from the center.
