22.1 EARLYCHILDHOOD CARE AND EDUCATION

Comments:
An unannounced renewal inspection took place on-site on 02/01/2022 and the inspection concluded on 02/03/2022 with a virtual record review inspection. There were 14 children, ages 3 years-4 years, and 6 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during learning/circle time, participating in music and movement and washing hands. A total of 5 children?s records, 5 staff records, 1 medication and 6 Board Officer/Agent Records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-40-K
Description: Based on discussion with staff, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma.

Evidence: Staff 2 stated that the center did not have written procedure for prevention of shaken baby syndrome or abusive head trauma.

Plan of Correction: Will discuss with management.

Standard #: 8VAC20-780-40-M
Description: Based on observations and discussion with staff, the center failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities and dietary restrictions documented in the allergy plan required in 8VAC20-780-60-A-8.

Evidence: Staff 2 stated that there was not a current written list of children?s allergies, sensitivities and dietary restrictions.

Plan of Correction: Will print the model form and complete it.

Standard #: 8VAC20-780-60-A
Description: Based on record review and discussion, the center failed to ensure that the separate record for each child enrolled contained all of the elements as required by the standards.

Evidence: The record for Child 3 did not contain a written care plan, to include instructions from a physician, regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. The record for Child 3 contained documentation of a diagnosed food allergy.

Plan of Correction: Will get the form to the parent.

Standard #: 8VAC20-780-80-A
Description: Based on record review and discussion with staff, the center failed to maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs for each group of children.

Evidence: The written record of daily attendance for each group of children did not document the departure of each child as it occurred.

Plan of Correction: Will revise the form to include departure.

Standard #: 8VAC20-780-550-B
Description: Based on the review of policy/procedures and discussion with the staff, the center failed to ensure that the emergency preparedness plan shall contain procedural components for a lockdown.

Evidence: The emergency preparedness plan reviewed during the 02/01/2022 inspection did not include a component for lockdown.

Plan of Correction: Will discuss with management.
