Comments:
This inspection was conducted by licensing staff using a hybrid protocol, including an onsite inspection of the child day center and virtual review of documents.

A renewal inspection was initiated on January 13, 2022 and concluded on January 14, 2022. The licensing inspector was onsite from approximately 10:00am until 11:30am. There were 54 children present, ranging in ages from four months to five years with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four children records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of children's records, the center did not obtain all required documentation for each child's record.

Evidence:

The record of child #3, with a diagnosed food allergy, did not have an allergy care plan on file.

Plan of Correction: Per the director: I will ask the parents to provide an allergy care plan for the child's record.

Standard #: 8VAC20-780-280-B
Description: Based on observations, the center did not ensure Hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.

Evidence:

1. The laundry room door was observed opened with the key in the door knob. in the laundry room, cleaning products were sitting on the low counter. 2. The kitchen door was open and cleaning products were observed sitting on the counter and table.

Plan of Correction: Per the Director: I will lock the closet door and put the cleaning materials in the kitchen away in a locked cabinet.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure all prepared infant formula was dated and labeled with the child's name.

Evidence:

Bottles for Child #4 were observed in the refrigerator. The bottles were labeled with the child's name but did not have the date on them.

Plan of Correction: Per the director: I will review with infant classroom staff our policy to label any bottles that arrive unlabeled each day.
