A complaint inspection was conducted in response to several complaints received by the Office of Childcare Health and Safety related to ratios, food service and physical plant. Violations related to this complaint are on the attached violation notice.

Standard #: 8VAC20-780-350-C Complaint related: Yes Description: Based on observation, it was determined that the facility did not ensure that when children were in mixed aged groups, the ratio to the youngest child is applicable.
Evidence:
There was an ongoing mixed age group of children during the inspection that contained 19 children ages two through school age. Since the youngest child was two the required ratio of 1 staff for every eight children. There were only two staff present with this group. An additional staff member was required.Plan of Correction: Facility will ensure that adequate staff are available .

Evidence:
The posted menu was from August.Plan of Correction: Director will ensure that the current week's menu is posted where it can be seen by the parents.
