Comments:
This inspection was conducted by licensing staff using an alternate hybrid protocol, including telephone contacts, documents review, interviews and onsite observations of physical plant and programing.

A renewal inspection was initiated on February 24, 2022 and concluded on February 24, 2022. The director was present for the onsite inspection. There were 25 children present, ranging in ages from 16 months to five years, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The director submitted documentation and records virtually for review. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A-1
Description: Based on review of staff records, the center did not obtain documentation of a tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence:

The record of staff #2, hired and working with children as of 02/03/2022, did not contain documentation of a negative tuberculosis screening. Documentation is required to be submitted at the time of employment and prior to coming into contact with children.

Plan of Correction: Per the director: Staff #2 had a tuberculosis screening done yesterday and should receive the results by tomorrow. In the future, I will ensure all results are obtained at hire.

Standard #: 8VAC20-780-240-E
Description: Based on staff record review and interview, the center did not ensure staff completed orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment.

Evidence:

The record of staff #1, hired 01/07/2022, did not have documentation of completion of first aid and CPR orientation training within 30 days of hire.
2. The director stated CPR & first aid orientation had not been conducted with staff #1.

Plan of Correction: Per the director: I will cover CPR & First aid training at the time orientation is conducted going forward. I will review with staff #1. Staff #1 is scheduled to complete CPR & First aid certification in two weeks.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.

Evidence:

A container of bleach was observed in a unlocked low cabinet in the kitchenette/ supply area. The director stated the children line up at the door of the kitchen area while lunches are being warmed creating possible access to the unlocked cabinet.

Plan of Correction: Per the director: I will purchase a child safety latch for the cabinet where the cleaning product is stored. I will move the bleach to a locked cabinet until I have the safety lock.
