Comments:
This complaint inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and staff interviews. A complaint was received by the department on 11/30/2021 regarding supervision. A complaint inspection was initiated 12/01/2021. Additional information was obtained from the facility on 1/28/2022.
The director was contacted by telephone to conduct the investigation and staff interviews were conducted. The evidence gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Complaint related: No
Description: Based on interviews with staff and review of supervision policies, the facility failed to follow the center's own policies and procedures that are required by these standards. Evidence: the center's supervision policy states, "Each staff member will count and confirm with other staff members the number of students in their care upon transitioning from playground/fellowship before leaving the area". On November 30, 2021, Child A (age 3 years) was out of sight and sound supervision, after being left alone on the playground when the class returned to the building.

Description: Based on staff interviews, the facility failed to ensure that when staff are supervising children, they always ensure their care, protection, and guidance. Evidence:
1. Around 4:30 p.m. on November 30, 2021, Child A (age 3 years) was seen alone on the playground by a preschool parent, who was of no relation to the child. The preschool parent rang the doorbell and let an employee of a separate child care program know that the child was outside alone.
2. The employee of the separate child care program brought the child in from outside and stated the child was "shy and scared".
3. The employee of the separate child care program returned the child to his group, who were now playing in the fellowship hall.
4. The preschool staff were not aware that the child was not with the group until he was returned to the group.
5. The employee of the separate child care program stated that the class had been inside for a minimum of five minutes.

Description: Based on staff interviews, the facility failed to have all children under 10 years of age within actual sight and sound supervision. Evidence: From approximately 4:30 p.m. to 4:36 p.m. on November 30, 2021, Child A (3 years) was alone on the playground, out of sight and sound supervision of staff who had returned to the building. A preschool parent, of no relation to the child, rang the doorbell and told an employee of a separate child care program that the child was alone on the playground. The employee from a separate child care program brought the child inside and returned him to his group. The child was out of sight and sound supervision for a period of at least five minutes.
