Transportation was not observed today. A review of transportation related standards, to include 580-C-5 and 550-K, were discussed regarding transportation and required items to be on the vehicle when transporting children were reviewed.
- Comments:

This inspection was conducted by licensing staff, in part, by using an alternate remote protocol, including telephone contacts, document review and interviews. A tour of the program was also conducted.
A Renewal inspection was initiated on 2/22/2022 and concluded on 3/4/2022. The provider was contacted by telephone and documentation was reviewed virtually. On 2/22/2022 there were 22 children and 3 caregivers present at the time of the inspector's arrival. Two staff records and two children?s records were reviewed. The inspector reviewed requirements and compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-2 Description: Based on observation, interview and record review, the licensee did not obtain a Criminal History Record Check for every staff prior to their 1st Day of Employment/ Service.
Evidence:
1. During the inspection on 2/22/2022, staff #2 was alone in the classroom with 7 children, ages 2 to 3 years old.
2. The record for staff #2, which indicated that staff #2 began service 1/28/2022, did not contain the results of a Criminal Record Search.
3. When the administrator was asked for the Criminal Record Check for staff #2, she stated that this individual was a volunteer, therefore she had not obtained background checks for her.
4. If an individual is counted in the ratio of staff to children they are considered staff and are required to obtain all of the required record documentation of a staff person (including background checks) regardless of whether they are compensated for service or not.Plan of Correction: The licensee stated that she was not aware that this individual was required to have background checks because she is not a paid employee. However, now that she is aware she has let staff #1 know that she may not work in ratio with the children again until all required background checks results are received.
All center staff, whether paid or not, will have background checks prior to working at the center.
An appointment was made for staff #2 to obtain fingerprinting for the Criminal Record Search and the results will be filed in the employee's record upon receipt.
Standard #: 8VAC20-770-60-C-2 Description: Based on observation, interview and record review, the licensee did not obtain Central Registry Results by the end of 30th Day of Employment/Service for every staff.

Standard #: 8VAC20-780-160-A-1 Description: Based on Observation, Interview, and record review, the licensee was unable to demonstrate that documentation of the results of a Tuberculosis screening was submitted for every staff at the time of employment and prior to coming into contact with children.
Evidence:
1. During the inspection on 2/22/2022, staff #2 was working in a classroom with 7 children, ages 2 to 3 years old.
2. The record for staff #2, which indicated that staff #2 began service 1/28/2022, did not contain the results of a Tuberculosis screening.
3. When the administrator was asked for the Tuberculosis screening results for staff #2, documentation was provided which indicated the date that the Tuberculosis test was administered. However, there were no results of the test or screening documented on the form or in the record.Plan of Correction: The administrator stated that she has let staff #1 know that she may not work in ratio with the children again until all required documentation, to include the results of a tuberculosis screening, are received.
All center staff will have Tuberculosis screening results documented in their record prior to coming in contact with the children in care.
