Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on February 10, 2022 and concluded on February 10, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 116 children present, ranging in ages from two years to six years, with 21 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on a review of five staff records and interview, the center did not ensure that one out of five staff obtained fingerprint results prior to hire.

Evidence:
1. The record for Staff #3, hired on 08/30/21, did not contain a fingerprint-based background check determination letter from the Office of Background Investigation prior to employment.
2. During interview, a member of management confirmed the fingerprint-based background check results for Staff #3 were not received at the time of training.

Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, the center did not ensure to obtain the results of a tuberculosis screening for each staff within the required time frame.

Evidence:
1. The record of staff #1 (start date: 09/07/21) and staff #3 (start date: 09/09/21) did not have documentation of a negative tuberculosis screening. 2. The Director acknowledged that the screening was missing. 3. The record for staff #5 had a TB screening dated 03/24/2015.
