This inspection was conducted by licensing staff using an alternate remote protocol with an in-person component that included telephone contacts, virtual and in person documents review, interviews and a tour of the program with observations conducted at the center.
A renewal inspection was initiated on 2/14/2022 and concluded on 3/8/2022. The director/provider was contacted by telephone and a virtual inspection was initiated to review records. The in-person component was completed on (date). There were 32 children present, ranging in ages from infants to five years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on an interview with the program director and record reviews of documents, the center failed to ensure that all persons who indicate on a sworn statement having lived outside Virginia in the last five years, and who work alone with children, have completed all required background checks prior to employment.
Evidence:
1. There was no out-of-state sex offender background check conducted for staff #2 prior to hire date of 10-25-2021 and staff #4 prior to hire date of 7/1/2021.
2. There was no out-of-state central registry background check sent for staff #2 and #4 as of the date of inspection.Plan of Correction: The director will ensure that all staff hired have the correct background checks within the set time frames.
Standard #: 8VAC20-780-160-A-1 Description: Based on record review, the center failed to ensure that documentation of a negative TB screening was submitted at the time of employment and prior to coming in contact with children.
Evidence:
1. Staff #2 has a hire date of 10/25/2021 and the TB test/screening on file is dated 2/16/2022.Plan of Correction: Director will ensure that all staff hired have a TB test/screen at time of employment or prior to coming in contact with children.
Standard #: 8VAC20-780-540-B Description: Based on observation the center failed to ensure that the first aid kits are not accessible to children.
Evidence:
1. Both first aid kits located on the first and second floors were unlocked and accessible to the children. The first aid kits have bottles of Peroxide and Activated Charcoal.Plan of Correction: Corrected during the inspection.
Standard #: 8VAC20-820-120-E-2 Description: Based on observation the center failed to have the most recent violation notice posted.
Evidence:
1. On 2/14/2022 the center did not post the most recent violation notice, dated October 13, 2021.Plan of Correction: Corrected during inspection.
