Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and an on-site tour of the program.

A monitoring inspection was conducted on 3/14/2022. Parts of this inspection were completed virtually, and parts were completed on-site. There were 74 children present, with 12 staff directly supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies. A total of 4 children's records and 3 staff records were reviewed.

The children were observed creating art projects, playing outside, enjoying classroom creative play activities, and engaging in handwriting development projects.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on a review of children's medications, it was determined there was not a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
The record for Child E contains documentation indicating that the child has a diagnosed food allergy. There is no food allergy action plan in the child's record.

Plan of Correction: The parent will be asked to have an allergy action plan completed and signed by the child's doctor.

Standard #: 8VAC20-780-330-B
Description: Based on observations, it was determined there is not enough resilient surfacing in various fall zone areas of the playground. In addition, some of the resilient surfacing has not been maintained in proper repair.
Evidence:
1 - There is not enough mulch at the bottom of one of the slides and beneath the stairs leading to the top of the playground structure. In these areas the mulch has worn down to dirt.
2 - The permanent rubber surfacing which is at the bottom of the main slide has torn and ripped in various places.

Plan of Correction: I will contact maintenance to rake the extra mulch into areas where it is needed. I will also ask maintenance to repair the rubber surfacing.
