63.2 Child Abuse & Neglect

Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:00 am-11:30 am. There were a total of 56 children with 6 supervising staff members present. Children were observed engaged in centers, circle time, and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1. Staff #4 (Date of hire-11/8/2021) did not complete the Virginia Department of Education sponsored orientation course within 90 days of employment.
2. Staff #3 confirmed that Staff #4 had not completed the orientation course within 90 days of employment.

Plan of Correction: Staff will complete the orientation course by April 1st.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Primary 1 classroom, a bottle of sanitizing solution and a bottle of hand sanitizer was located on a high shelf and not locked up.

Plan of Correction: The sanitizing solution and hand sanitizer was immediately taken from the high shelf and locked up in a cabinet. All hazardous substances will be locked up.

Description: Based on record review and interview, the center did not develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
Evidence:

2. Staff #3 confirmed that there was not a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff members.

Plan of Correction: A written plan and procedure will be developed and implemented.
